CENTRALIZED REGISTRATION

All new students entering Margaretta Local Schools must be registered through the Board of Education office. Please call 419-684-5322 to schedule a registration appointment with our Central Registrar, Trish Simecek.

ENROLLING A NEW STUDENT?

• To avoid delays, the Registration Packet should be completed prior to your registration appointment.
• All required documents must be submitted at the registration appointment. We will be unable to enroll your student(s) until these documents are submitted (including proof of residency and custody documents).
• Students must be registered by the custodial parent or legal guardian.

DOCUMENTS REQUIRED FOR REGISTRATION

• Registration Packet completed prior to your registration appointment
• Photo identification of the custodial parent or legal guardian registering the student
• Student's original or official birth certificate
• Student's social security card
• Authorization for Record Release (included in Registration Packet)
• TWO FORMS of proof of legal residency - utility bill, credit card bill, bank statement, employment forms or any current official or legal document that includes the name and street address of the custodial parent or legal guardian. Without proof of residency within the Margaretta School District, tuition will be charged.
• Emergency Medical Authorization (will be sent home the first day of school or will be included in Registration Packet, depending on when student enrolls)
• Immunization Record
• Ohio Health History (Kindergarten only)
• Dentist's Report (Kindergarten only)
• Physician's Report (Kindergarten only)
• Copy of last report card or unofficial transcript (not required, but will facilitate scheduling)

DOCUMENTS REQUIRED, IF APPLICABLE

• Custody documents - proof of legal custody including a certified copy of an order or decree allocating parental rights and responsibilities for the care of a child and designating a residential parent and legal custodian of a child.
• Additional legal forms - proof of Legal Custody in Process, Grandparent Power of Attorney, Caretaker Authorization Affadavit, etc.
• IEP, ETR/MFE/Section 504 for students with disabilities (not required, but will facilitate scheduling)

Please bring originals of all required documents. Copies will be made during the registration appointment.
